This project is a test of the new forms / survey system, but there is an award available for participating.

When you create a form you can add fields for users to fill out and the resulting data can be downloaded as a CSV for use in Excel or other software.

You can also choose to either award the prize to a random responder, to evenly split the prize with all responders, or you can manual select the winner(s).  The prize in this particular project will be awarded to a random responder.

Please fill out this survey so I can find out what you all think of this system.  Remember - you could win the entire prize!  Please fill out all fields (except "Other" unless appropriate).  Any incomplete responses will be removed.  Thanks!
